Building a Life Lesson Form
In WordPress, under Tools, select Formbuilder.
Create a new form or make a copy of a pre-made form*.
*If you choose this option, make sure you make changes to the name, subject line, thank you text, and questions.
FORM CONTROLS
- Name – The title of your lesson. For convenience and ease, title it similar or identical to the life lesson page.
- Subject – Type ‘Life lesson: subject or title of lesson’

Recipient – Add emails to whomever should receive the forms. Always use the general email – forms@truthmedia.com.

Method – Select POST

Action – Select ‘Form to email’

Thank you text: Add something like ‘Thank you, your study question responses have been submitted.’ If it is part of a series add, ‘Return to the lesson series <a href=”url to series lesson index page”>Title of series</a>.

Autoresponse: Select ‘Life lesson autoresponse’
- Form tags: “life lesson”
FIELDS
This is where you add the study questions and contact information questions for the life lesson.
Lesson questions
You can add as many questions as you need to. Generally, we don’t like to add more than 10 for the readers sake. For lesson questions:
- Field Type – Select ‘large text area’
- Field Name – Type in the question (when you save it, it will input underscores instead of spaces but just ignore that). Remember to add the number of the question (ie. 1, 2, etc.)
- Field Value – leave blank
- Field Label – Again, type in the question with the number in front. Be careful of the length. Some questions may need to be shortened to fit in this field.
- Required Data – leave blank
- Error Message – leave blank
- Help Text – leave blank unless you would like to add more thoughts to the question.
Contact information
After you have added all of the questions, you will need to add some other necessary contact questions.

- Field Name – Type ‘Name’
- Field Value – leave blank
- Field Label – Type ‘Name’
- Required Data – Select ‘any text’
- Error Message – Type ‘You must enter your name.’
- Help Text – leave blank

- Field Type – Select ‘single line text box’
- Field Name – Type ‘Email’
- Field Value – leave blank
- Field Label – Type ‘Email’
- Required Data – Select ‘any text’
- Error Message – Type ‘You must enter your email address.’
- Help Text – leave blank
Confirm email:
- Field Type – Select ‘single line text box’
- Field Name – Type ‘Confirm Email’
- Field Value – leave blank
- Field Label – Type ‘Confirm Email’
- Required Data – Select ‘any text’
