Discussing My Affair With My Wife

Can you give me some suggestions for safely discussing the issues which existed in my marriage prior to my extramarital affair? While I take full credit for my actions, there was a lack in our marriage which caused me to seek attention elsewhere. I would like to address the problems as we rebuild. It seems that each time I try to broach these items with my wife, she becomes extremely defensive.
